Can someone please tell me what is included in the box?

As I recall, the box includes the amplifier and a power adapter. Notably, it does NOT include a cord connecting the power adapter to the amplifier! You will either need to make a cable using some RG-6 and coax connectors or use an already existing cable cord.

Would this amplify an OTA signal or not?

This would help with signal lost due to distribution: long cable runs, excessive splitting, etc. If your antenna is not catching a good signal, this won't help. You would need a better antenna.
