Are the Keller extension ladder rungs the same diameter as a Werner to where I can purchase a new rope pulley and have it work without modification?

It is likely that you will be able to use the replacement pulley on your Keller ladder, but not 100% sure. It is the same part for all Werner Ladders. The Amazon part # B00JAGEE9S is a Werner Rope and Pulley kit.

Will this work on an old aluminum ladder that never had a prior pulley assembly?

If it's an extension ladder and you need to replace the pulley wheel it should work. Replaced the pulley wheel on mine because the nylon wheel wore out. This pulley works fine on my extension ladder.

what thickness of rope does this take? 5/16, 7/16, or 3/8

They say 6/16. But im sure slightly larger work also, because there is enough room for 7/16 and 3/8 is still enough contact to move the wheel.
