I have small hands and also wrist pain problem from traditional mouse. Is this a good option for me?

For anyone seeing this later, this is a good fit for medium hands (6.7" hand length+). Logitech has a small version called the Logitech Lift which is about 2.7"L and 2.7"W vs the Anker which is 5"L and 2.5"W. The length here would be more from the crook of the thumb to the end of the hand, so 5" is pretty good. It might be a bit small for big hands. So for small hands look for something more like the size of the Logitech Lift.

Does this work with OSX Mavericks? Is there a work around to get the thumb buttons to work?

So, I just bought this on 8/2020. I'm on a mac and the back/forward thumb buttons are working normally out of the box; I didn't do anything. I knew they weren't suppose to, but perhaps Anker fixed this?

does this mouse recharge batteries if you can connect a usb cable?

No, but this thing does not use that much power even at 1600 dpi, I change batteries every 8-12 months . Buy a pack of amazon triple AAA and you'll have enough batteries for life. Great mouse I use for gaming (CODCW/BF4) and in the office for graphic design. Great mouse, not cheap at almost $30 but best in it's price range. Light weight rubbery feel.

Is the scroll wheel anywhere close to the low-friction free-rolling wheel in logitech mx mice? does it keep spinning on its own?

The scroll wheel is not "free rolling". It stops when you stop rolling it and appears to be set at intervals-isn't a smooth continuous scroll. Haven't used the Logitech MX so have no comparison to communicate to you.
