It's just an updated version (1080p 60fps) of DmC: Devil May Cry only with all the DLC from the original release plus extra game modes and costumes.

Is all the original DLC included on the disc, or is the DLC a code that you must redeem from the PSN?

All DLC is on the disc. There is no redemption code.

Can transfer my saved game from ps3 to the ps4 version?
