Can the GoPro attachment be added to the stock Garmin out front mount?

Unfortunately, the Quad Lock GoPro attachment is designed to fit the Quad Lock Out Front Mount only.

does this come with universal adapter or do i have to buy one to stick to the back of my phone separately

It does *not* come with the adapter that would attach to an existing phone or phone case. You can buy the phones cases with integrated adapter separately or the universal adapter to attached to your phone or phone case (if flat on the back). Most of the other cheap, inexpensive phone mounts are crap. This one is solid. http://www.amazon.com/Annex-Universal-Adapter-Equipped-Devices/dp/B008O0Z9YU/

Does this fit handlebars that are 25.4mm?

Yes it does, the Out Front Mount suits handle-bar sizes that are either 31.8mm or 25.4mm

Would this fit my Yamaha fJR?

The following should provide the answer to your questions: Heads up position provies better viewing angle Patented dual-stage locking mechanism ensures your phone is held securely to your bike Can be rotated for over stem mounting Suits handlebar sizes 31.8mm and 25.4mm Compatible with all Quad Lock Cases. Optional GoPro Adaptor also available (not included) Since I have no way of knowing the handle bar size on your Yamaha, I can't definitely tell you it will fit. There are two sizes mentioned above, and I would think that it will fit since handle bar size is relatively consistent among all manufacturers.
