I would say it's standard size, maybe a little smaller than the desktop plug-in mouses. But definitely not miniature! It's a good size for throwing in a bag and not needing a ton of extra space

Does this take any batteries and what size and how many?

Yes two AAA and I have had them in for 6 months now and I use it everyday.

Does this mouse feature a tilt-wheel?

The wheel does not tilt. It scrolls up and down. If you press down on the wheel, it goes into the scroll mode where you move the mouse to scroll.
